FEROZEPUR MEDICITY HEALTH CARD – Terms & Conditions

1. Membership Eligibility:

• Only the first family members, including the husband, wife, and up to 3 dependent children, are eligible for enrollment.
• For enrolling more than five members (first family members only), an additional fee of Rs. 500/- per member, up to two extra members, is applicable.

2. Documentation:

• Submission of a copy of Aadhar Card for all family members is mandatory for card enrollment.

3. Membership Duration:

• The FMC HEALTH CARD is valid for one year from the date of issue.

4. Non-Transferable:

• The card is non-transferable under any circumstances.

5. Membership Fee:

• The membership fee is non-refundable under any circumstances.

6. Card Usage:

• The FMC HEALTH CARD is not a payment card or a proof of creditworthiness. Any attempt to use it as such may constitute fraud.
• An Aadhar Card, along with the FMC HEALTH CARD, is mandatory during OPD visits.

7. Record of Visit:

• Upon presenting the FMC HEALTH CARD at the hospital, the staff will issue a record of visit slip to the cardholder or make a log entry.

8. Card Ownership:

• The cardholder is solely responsible for maintaining the FMC Health Card. Ferozepur Medicity Super-Speciality Hospital will not be responsible for damage, theft, or any unforeseen circumstances.

9. Lost or Damaged Cards:

• In case of loss, theft, or damage to the FMC HEALTH CARD, notify Ferozepur Medicity Super-Speciality Hospital immediately. A replacement fee of Rs. 150/- will be levied for each replacement.

10. Cancellation and Renewal:

• In the event of the cardholder canceling or not renewing their FMC HEALTH CARD, the card shall be invalid from the cancellation date or expiry date. Any hospital visits made by a cardholder using an invalid card will be charged to the cardholder.
• Renewal terms and conditions are at the discretion of FMC HEALTH CARD, Ferozepur Medicity Super-Speciality Hospital.

11. Exclusions:

• Visiting doctor’s OPDs are not covered by the FMC HEALTH CARD.

12. General Provisions:

• The terms and conditions are subject to change, and any updates will be communicated to the cardholders through appropriate channels.

By enrolling in the FMC HEALTH CARD, you acknowledge that you have read, understood, and agreed to abide by these terms and conditions.
